Are you being ‘Cared For’?
Does a relative, friend or neighbour help to look after you because you are ill, frail, disabled, have a mental health concern or problematic substance use and you could not manage without their help? If so, then you are being ‘Cared For’ and are being supported by an unpaid carer.
Please take a few moments to read this note and complete the form on the reverse of this sheet.
Why you should complete the ‘Cared For’ Patient Consent Form
The person who cares for you (known as your unpaid carer) may have been given a separate Unpaid Carer GP Registration Form to complete. However, this does not give the person who cares for you the right to ask about your health care. This is because of data protection rules that state you have to consent to your personal details being shared with your unpaid carer.
However, if you complete and sign this form you are giving the GP surgery permission to record you as a ‘Cared For’ patient. You will also be identifying your unpaid carer and giving permission for them to discuss your personal care with members of your health care team. This is particularly useful should an emergency arise.
What happens when you hand the completed form back to a member of surgery staff?
Once the member of surgery staff has checked you have completed everything, you and your unpaid carer will be asked to sign the bottom of the form. The relevant member of staff will ensure the information is recorded on the surgery’s patient record system.

Pam y dylech lenwi’r Ffurflen Caniatâd Claf sy’n ‘Derbyn Gofal’

Funded by Welsh Government, the Short Breaks Scheme is a new initiative to support unpaid carers of all ages in Wales to take a break from their caring role. A short break is more than just a yoga class or an overnight stay in a hotel, it is a chance for carers to de-stress and recharge. It offers a break from the daily challenges of caring for a family member or friend.
Bowel cancer is the third most common cancer in Wales. Bowel screening aims to find cancer at an early stage when treatment is likely to be more effective. Early detection is key. At least 9 out of 10 people will survive bowel cancer if it is found and treated early.
Bowel screening is currently offered to people who are aged between 55 and 74 and live in Wales. We get address information directly from GPs. It is important that the person’s correct home address is registered with the GP. We do not hold any medical information.
Benefits of Screening
As a carer you may have your own personal views about taking part in screening. Your view may differ from that of the person you care for and you may feel that the person you care for is not making the ‘right’ decision.
You must remember that your role is to provide the person with the correct information (in an appropriate format) and support, so that they can make an informed decision to take part in screening or not. The form below may help you to provide this informed information.

The advisers who work on the helpline have a good understanding of the challenges facing young carers and can provided information and advice on their rights etc. In addition, they can support them to access specialist young carer services, as well as advocate on their behalf to professionals involved in their lives.The Meic website also includes lots of interesting articles and has a professional section with lots of useful resources. They have lots of engagement with children, young people and professionals on their social media pages on Instagram, Facebook and Twitter and you can find some of the videos they’ve created on their YouTube channel.Meic operate between 8am and midnight every single day of the year.
Listening Support Service – a free, telephone service providing emotional support to unpaid carers, offering them a confidential, safe space to talk about the challenges of caring. We offer a series of up to 4 calls, with the same caller at the same day & time each week, for around 30 minutes. It’s a chance to share with someone who understands, but isn’t connected to the situation.

Does this sound like you? If so, you are an Unpaid Carer. We are interested in identifying Carers, especially those people who may be caring without help or support. We know that Carers are often “hidden” looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a Carer.
We feel that caring for someone is an important and valuable role in the community, which is often a 24-hour job that can be very demanding and isolating for the Carer. We further believe Carers should receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits and respite care and not least, a listening ear when things get too much.
Unpaid Carers provide around 97% of the Care in the community and save the NHS in Wales over £8.1 billion each year. St Thomas’ Surgery recognises that Carers contribute a significant saving to the NHS by looking after relatives, neighbours or friends who might otherwise have to go into long term care. Young Carers, under the age of 18 may be particularly vulnerable due to their inexperience and lack of life-skills. Young Adult Carers, between the age of 18-25, can be the most hidden group of Carers in our society. Therefore we are committed to providing appropriate support and consideration to its patients who are Carers whatever their age.

As a Carer, you are entitled to have a Carers Needs Assessment by Social Services. It is a chance to talk about your needs as a Carer and the possible ways help could be given. It also looks at the needs of the person you care for. This could be done separately, or together, depending on the situation. There is no charge for an assessment. Every Carer is entitled to an assessment; however it is not an assessment of your ability to provide care. When you register with us as a Carer, the practice will offer you the chance of a referral.
