Patient Registration

St Thomas' Surgery

St Thomas Green

New Patient Medicals

All patients are asked to complete a New Patient Questionnaire when registering so we can better understand your medical situation.  This will then be reviewed by one of our Health Care Assistants who, where appropriate, may contact you to discuss your responses or ask you to come in to the Surgery for a face to face review.

Register As A Patient

If you wish to register with the Surgery, you will need to collect the relevant registration forms from Reception.  Patients will be registered with the practice rather than an individual doctor.  However, when booking appointments you may express a preference of which doctor you see depending on appointment availability.

Temporary Patients

You are able to see a doctor anywhere in the UK, if you are away from home and in need of medical help.  You can do this by simply asking to see the nearest doctor, as a temporary resident.  We will be happy to see any relatives or friends staying with you, if they need a doctor.

Chaperone Policy

As a Practice we are committed to providing a safe, comfortable environment for our patients.  If you would like a chaperone during your consultation, please ask the doctor.  A Practice Nurse or Health Care Assistant will usually act as your chaperone, but on rare occasions a Receptionist may be asked to chaperone.
