GP Record:

NHS Wales has changed the way it uses patient information for care.

Mae crynodeb digidol o'r wybodaeth a ddelir yng nghofnod eich meddyg teulu ar gael i'r meddygon, y nyrsys a'r gweithwyr iechyd proffesiynol cofrestredig a fydd yn gofalu amdanoch tra byddwch yn yr ysbyty. Bydd yn eu helpu i ddarparu gofal cyflym, diogel ac o ansawdd i chi.

Mae gwybodaeth hanfodol o gofnod eich meddyg teulu wedi bod ar gael ers amser i gefnogi eich gofal pan fo'r feddygfa ynghau ‘y tu allan i oriau’, neu mewn argyfwng.

Fodd bynnag, hyd yma nid oedd y rhai a oedd yn gofalu amdanoch yn yr ysbyty, neu yn ystod apwyntiad cleifion allanol neu arhosiad cleifion mewnol, yn gallu mynd ati ar unwaith i gyrchu'r wybodaeth bwysig a ddelir gan eich meddyg. Mae hyn yn cynnwys meddyginiaethau cyfredol, profion diweddar ac alergeddau.

Only the healthcare professionals looking after you can see your information, and only with your permission. NHS Wales can check on who, when and how every record was looked at.

The Welsh GP Record is a summary of your full GP medical record. It contains your:

• Name, address and contact details

• Current medication and medication they have been prescribed in the last two years

• Allergies or any adverse reactions

• Current problems or diagnosis

• Results of any recent tests they have had in the previous year, for example, blood tests and x-rays

Nid yw'n cynnwys unrhyw drafodaethau preifat y gallech fod wedi'u cael â'ch meddyg teulu

Consent opt-out:

As a Practice we process information based on the lawful basis for processing which includes, but is not limited to, consent.  We therefore may not need explicit consent for every instance of processing and sharing your information.  Where the basis of consent is used, a patient can opt to withdraw consent.  If you don’t want anyone other than your GP to see your records you can opt-out. You can do this by completing an opt-out form available from your GP practice. Patients are encouraged to talk with their GP before opting-out as it could make a difference to the care they receive. If a parent or guardian of a child under 16 does not wish their child to have a record, they can opt-out on their behalf, but should discuss this with their GP first.
