If so, then you are being \u2018Cared For\u2019 and are being supported by an unpaid carer.<\/span><\/p><p style=\"font-weight: 400;\"><span style=\"color: #3d4459;\">Please take a few moments to read this note and complete the form on the reverse of this sheet.<\/span><\/p><p style=\"font-weight: 400;\"><span style=\"color: #3d4459;\"><strong>Why you should complete the \u2018Cared For\u2019 Patient Consent Form<\/strong><\/span><\/p><p style=\"font-weight: 400;\"><span style=\"color: #3d4459;\">The person who cares for you (known as your unpaid carer) may have been given a separate <em>Unpaid Carer GP Registration Form<\/em> to complete. However, this does not give the person who cares for you the right to ask about your health care. This is because of data protection rules that state you have to consent to your personal details being shared with your unpaid carer.\u00a0<\/span><\/p><p style=\"font-weight: 400;\"><span style=\"color: #3d4459;\">However, if you complete and sign this form you are giving the GP surgery permission to record you as a \u2018Cared For\u2019 patient. You will also be identifying your unpaid carer and giving permission for them to discuss your personal care with members of your health care team.
